"Burden of occupational diseases treated in the Spanish National Health System" a càrrec de Consol Serra

The identification and recognition of occupational diseases (OD) has important and longstanding limitations in Spain. The consequences of under detection are varied; among these, the fact that medical costs are borne by the National Health System instead of the Social Security one. A new initiative to overcome this was the creation of an Occupational Disease Unit (ODU) at the Parc de Salut Mar health system (PSMAR) in 2010. Its aims are to identify, evaluate and report suspected OD among PSMAR patients, based on the collaboration of PSMAR clinical services, its occupational health service and the patient. Between 2011 and 2014 a total of 115 suspected cases of OD were referred to the ODU, of which 46 were confirmed by the ODU occupational physicians after more in-depth evaluation. Of the 31 patients who elected to pursue official recognition of their OD, 13 have been successful and several others are in progress. These results show that the ODU can be useful and effective. The identification and evaluation of possible cases of OD in a hospital has benefits for patients, families, the treating hospital and the general population through prevention, but it entails a complicated and not entirely well understood recognition process that needs guidance from all professionals involved. The challenge of the project is to help identify and properly manage the burden of OD treated in a group of hospitals of the Spanish National Health Service. To expand this experience, 9 Spanish hospitals were recently invited to participate in a prospective clinical study, to: 1) estimate the underreporting of OD in their respective hospital populations; 2) evaluate the cost of OD treated in their institutions; 3) develop policies and procedures for the systematic evaluation of a suspected OD; and 4) develop guidelines to assist patients/workers, health professionals (clinicians and occupational physicians), lawyers and unions in the process of recognition of OD by the Social Security system. Project outcomes will be used to increase the capacity of hospitals to detect OD in Spain, improve the methodology for the confirmation of suspected cases, clarify the recognition process of OD and facilitate economic relationships between the Spanish national health and social security systems, for fair reimbursement of expenses related to the care of patients with a recognized occupational disease.

Licenciada y Doctora en Medicina, especialista en Medicina del Trabajo y en Medicina Preventiva y Salud Pública, y Máster of Science in Occupational Medicine (London University).Tiene la acreditación de Investigación y de Profesorado Lector y Colaborador, del Ámbito de Ciencias Médicas y de la Salud de la Agencia para la Calidad del Sistema Universitario de Cataluña (AQU). Es jefa del Servicio de Salud Laboral del Parc de Salut MAR de Barcelona (Hospital del Mar y otros centros), y profesora asociada de la Universidad Pompeu Fabra (UPF), investigadora del Centro de Investigación en Salud Laboral (CiSAL) y jefa de Estudios de la Unidad Docente de Medicina del Trabajo "Mateu Orfila" UPF (CiSAL UPF). Su actividad científica está centrada en la investigación relacionada con la Salud Laboral y la Salud Pública, incluyendo las enfermedades profesionales, la patología osteomuscular y la incapacidad laboral asociada, la valoración de la capacidad funcional para trabajar por razones de salud, las revisiones sistemáticas como herramienta para la salud laboral basada en la evidencia, las intervenciones de retorno al trabajo, los riesgos laborales en el sector sanitario, el tabaquismo y la epidemiología del cáncer de origen laboral. Es directora de tesis y tesinas. Realiza su actividad docente en los Grados de Medicina y Biología Humana, Grado de Relaciones Laborales y Máster de Salud Laboral de la UPF, la Escuela Superior de Enfermería del Mar, y el Máster en Seguridad y Salud en el Trabajo: Prevención de riesgos laborales (UPC/UB/UPF), y desde 2010 es Adjunct Associate Professor in Epidemiology, Human Genetics & Environmental Sciences at the University of Texas School of Public Health, Houston (USA). Ha publicado más de 130 artículos científicos, ganado proyectos competitivos (Fondo de Investigaciones Sanitarias FIS y otros) como IP, ha realizado estancias en el extranjero en el National Cancer Institute (Bethesda, USA, Junio-Agosto 2003), y en la London School of Hygiene and Tropical Medicine (Londres, Reino Unido, Octubre 1986-Octubre 1988), y ha participado en proyectos internacionales, como el proyecto "Reducing sickness absence in Scotland - applying the lessons from a pilot NHS intervention" coordinado por el Prof. Ewan B. Macdonald (Glasgow, Reino Unido), y el estudio CUPID, coordinado por el Dr. David Coggon (Southampton, Reino Unido), ambos referenciados más adelante. Desde 2010, fruto de la colaboración entre el Parc de Salut MAR y el CiSAL UPF, coordina la primera unidad clínica hospitalaria en España para el diagnóstico y notificación de enfermedades profesionales, así como fomentar la investigación y la docencia en este ámbito. Desde 1999 contribuye a la Colaboración Cochrane, como coordinadora del Lung Cancer Review Group (1999-2003), como revisora y, desde 2010 como editora miembro del Occupational Safety and Health Review Group (COSHRG)/Cochrane Work (desde 2014). Ha sido representante española y actualmente miembro de la COST Action IS1002 Modernet. Ha sido representante española y presidente de la Sección de Medicina del Trabajo de la UEMS (1999?2013), y miembro de la Comisión Nacional de Medicina del Trabajo (Ministerio de Sanidad, Servicios Sociales e Igualdad) (2006-2014). Ha sido premiada, entre otros, con el Premio a la Excelencia Profesional por el Consejo de Colegios de Médicos de Catalunya y el Premio Ignasi Fina de Salud Laboral del Ayuntamiento de Barcelona.
